(1) Any false statement made in connection with this application to register a foal as a NY-bred may result in severe penalties, including prosecution.
(2) No horse can be registered as a NY-bred without first receiving certification from The Jockey Club.
(3) The name of the breeder must match The Jockey Club breeder information exactly.
(4) Award checks will be sent to the managing partner (the first party named as breeder).
(5) Award checks paid to the managing partner are for all partnerships under the same TIN.
(6) Only the managing partner (or a designee) to which a TIN has been assigned can register a horse using that TIN.
(7) Award checks are only valid for six months from the issue date, and must be cashed within that period.
